Tri-State Memorial Hospital & Medical Campus will host their annual free sports physicals on Tuesday, June 14, and Wednesday, June 15, from 4:00PM-7:00PM, at Tri-State Family Practice & Internal Medicine, located at 1119 Highland Avenue, Suite 10, Clarkston.

There is no appointment necessary; the clinic is open to anyone seeking a sports physical for their children. Tri-State Memorial Hospital asks that the child or accompanying parent bring the required physical form from the school that they attend or will be attending. Please include demographics (name, birthdate, and address) and have completed before arriving. General forms will also be available upon arrival. Children under the age of 18 must be accompanied by a parent or guardian.

“As a community owned and operated hospital, we are always looking for different ways to serve the area,” said Kelly Pease, Director of Family Practice & Internal Medicine. “We are happy to continue to do so through our well-received annual free sports physicals.”

Tri-State Family Practice & Internal Medicine requires all patients and visitors to wear masks throughout the entire clinic process. Social distancing will remain in effect. Rooms are sanitized between patients and follow proper cleaning protocols as outlined by the Centers for Disease Control and Prevention.
